Disc Injuries and Degeneration

Last week we talked about whiplash and the complications that come from it. This week we are going to focus on discs, which are commonly injured in motor vehicle accidents, lifting and other repetitive motions, as well as degeneration from wear and tear.

We’re also going to clarify the difference between disc bulges, disc herniations, slipped discs, and other descriptions you may have heard.

What is a Disc?

The proper name for these structures is an intervertebral disc. This name comes from the fact that discs are only found and serving as a connection between vertebra.

Besides connecting two bones in the spine together, they also act as shock absorbers to provide cushion, and allow us to move. If your spine was solid bone from top to bottom, we would be capable of much less movement.

The traditional way of describing discs that makes it easier to understand is comparing them to jelly donuts. Hard shell with a soft center. It is more accurate to say the outer layer of a disc is like a car tire, and the inner portion is like chewing gum.

The inner portion of the disc is called the nucleus pulposus. It is slightly softer and contains a high water content which is how it acts as a shock absorber. The outer layer is called the annulus fibrosus. This is made of alternating layers that serve to contain the nucleus in the center. These are strong rope like structures that interweave at varying angles to provide maximum strength.

Different Disc Injuries

All disc injuries refer to a breakdown of the outer layer, the annulus. Depending on the amount of damage, the squishy nucleus can leak out of the center.

Some of the common injuries are:

  1. Annular Tears
  2. Disc Bulges
  3. Disc Herniations
  4. Protrusion/Sequestration
  5. Slipped Discs

The first 4 are ranked in order of severity. A slipped disc is actually an older term that is no longer commonly used since discs do not “slip”.

An annular tear means there is damage to the fibers in the outer layer, but the center of the disc is intact. A bulge means that the nucleus is pushing outward. A herniation means that the damage to the outer layer is bad enough that some of the center can actually escape containment. A protrusion or sequestration means that the annulus is no longer intact whatsoever and chunks of the disc can even end up floating in the spinal canal.
